At MHMC, our case management team works closely with patients and their families to coordinate proper treatment plans during their stay with us. However, what is even more important is the planning, which goes into their discharge.
Our team is comprised of professional, experienced case managers and social workers. They assess, plan, implement, coordinate and evaluate available options and services. As patient advocates they work hard to find the best after-care options for our patients. By fostering relationships with sub-acute institutions, outpatient therapy services and home health care services on behalf of every patient, we enable a smooth transition and continuum of care. These important resources are paired in accordance with each patient's individual needs, enabling them to achieve ultimate health more effectively and efficiently.
